Description

Assist individuals served by Lumibility. to secure meaningful employment and to provide and/or arrange the necessary job supports to ensure success. Provide supervision, support and training to people with differing abilities served by Lumibility. Develop and implement activities that are person-centered and promote positive outcomes. Provide the environment and supports necessary to promote appropriate social skills and positive behaviors that will enhance skills, enrich lives, and foster community presence and participation.


Pay Rate: $23.33/hour

Hours: 36 hours/week (Tuesday-Friday 8am-6pm)


Essential Functions (not limited to):

  • Ensure the health, safety and well-being of people served by Lumibility in accordance with the individual service plan and in compliance with agency policies and procedures.
  • Meet employment needs of assigned caseload, including but not limited to: career exploration, job placement, career counseling, on and offsite coaching/support, and advocacy. Provide supports based on individual need to assist service participants to obtain and retain employment.
  • Assist and advise individuals on career development, including but not limited to: resume writing, interview preparation, and best career practices.
  • Train, and support individuals at employment sites.
  • Provide individualized follow-along supports to assist individuals in maintaining employment.
  • Develop and implement activities that are person-centered and promote positive outcomes.
  • Maintain medical, social and programmatic files by recording each person’s progress and documenting other required information. This includes, but is not limited to, service goals, attendance records, behavioral data, progress notes and other documentation as required or needed.
  • Ability to/Responsible for creating own schedule for individual caseloads to meet position hours with agreed upon core hours
  • Transport and/or assist in securing appropriate transportation for individual’s employment or community needs in accordance with Lumibility safe-driving policies and practices.
  • Ensure each individual on assigned caseload has a current and comprehensive electronic data base for all employment related information: assessments, work history, resume, job applications, interviews, positions, and employers.
  • Comply with DDS and other funder reporting requirements.
  • Comply with DDS and organizational training requirements.
  • Assist with coverage of community-based crews as needed.
  • Attend quarterly staff meetings in community setting after typical program hours.
  • Perform other duties as assigned.
Requirements
  • High school diploma or equivalent required; Bachelor degree in related field preferred.
  • Minimum of two (2) years of experience providing employment and/or transition supports to people with differing abilities.
  • Must be able to provide support and services utilizing community-based resources.
  • Must be over 18 and possess a valid Connecticut driver’s license, have an acceptable driving history and be able to utilize your own vehicle for Lumibility business as defined by Lumibility policy.
  • Ability to work flexible hours, and travel to varying locations as needed.

  • Knowledge of company policies and procedures and applicable regulatory requirements and standards.
  • Ability to read and understand policies and regulations and to translate general guidelines into specific action.

  • Skill in planning and organizing.
  • Skill in problem solving and decision making.
  • Skill in working with people with differing abilities.
  • Skill in operating standard office equipment, and agency data systems (Therap, Microsoft teams, Outlook etc.)
  • Skill in implementing programs and activities as outlined in the individual support plan.
  • Skill in communicating effectively in a courteous and professional manner.
  • Skill in writing clearly and concisely to maintain accurate records.
  • Ability to demonstrate good communication and interpersonal skills necessary to provide guidance and instruction to adults with significant challenges.
  • Ability to work successfully, both independently and in a team environment.
  • Ability to maintain confidentiality.
  • Ability to attend to the physical and personal needs of adults with differing abilities.
  • Ability to use adaptive and other equipment.
  • Ability to maintain a patient and positive attitude.
  • Ability to work flexible hours and travel to varying locations as needed.


REQUIRED PHYSICAL EFFORT:

Must be able to sit, stand and walk at an assigned location(s) and work for extended periods of time; bend/stoop, kneel and balance frequently; climb, reach above shoulder level; push/pull, squat and crouch occasionally; with assistance, lift, carry and position individuals of various weight. Must be able to lift and carry up to 50 pounds occasionally. Must be able to drive multi-passenger vehicles for extended periods throughout the day.
